Perspektivy prizmatické adaptace ve fyzioterapii neglect syndromu

| dc.description.abstract | Vzhledem ke ztrátě samostatnosti v každodenních činnostech je neglect jednou z hlavních příčin invalidity po cévní mozkové příhodě, přesto však neexistuje žádný všeobecně uznávaný rehabilitační přístup. Prizmatická adaptace je založena na efektu laterálního posouvání zrakového pole pomocí prizmatických čoček a jeví se jako perspektivní přístup v léčbě neglect syndromu. Tato práce se zabývá nejnovějšími poznatky a přístupy, popisuje možnosti konkrétních terapií a účinky této metody na pacienta. Byly prohledáno pět databází (Science Direct, PubMed Central, Nature, Taylor & Francis Online a Springer Compact) s 236 výsledky, které byly následně roztříděny a podrobněji zkoumány. Bylo zjištěno, že autoři používají pro hodnocení nejčastěji protokol Kessler Foundation Neglect Assessment Process, který používá škálu CBS a terapii dle Kessler Foundation Prism Adaptation Treatment, ze které vyplývá počet sezení mezi 8-12, síla hranolu 11,4°, počet ukazovacích pohybů 60 a celková doba rehabilitace 14 dnů. Pacienti vykazují zlepšení ve FIM (hlavně celkové a kognitivní) a ve skóre CBS, což naznačuje příznivý efekt na každodenní činnosti. Dále bylo zjištěno, že použití prizmatické adaptace může být prospěšná jak pro levostranný, tak pro pravostranný neglect syndrom. Metoda se dle zkoumaných studií jeví jako perspektivní, avšak výsledky jsou těžko porovnatelné kvůli nehomogenitě studií. | cs |
| dc.description.abstract-translated | Due to the loss of independence in daily activities, neglect is one of the main causes of disability after stroke, yet there is no universally accepted rehabilitation approach. Prismatic adaptation is based on the effect of lateral shifting of the visual field using prismatic lenses and appears to be a promising approach in the treatment of neglect syndrome. This study reviews the latest findings and approaches, describes the possibilities of specific therapies and the effects of this method on the patient. Five databases (Science Direct, PubMed Central, Nature, Taylor & Francis Online and Springer Compact) were searched with 236 results, which were then categorized and examined in more detail. It was found that the authors most commonly used the Kessler Foundation Neglect Assessment Process protocol for assessment, which uses the CBS scale and the Kessler Foundation Prism Adaptation Treatment, resulting in a number of sessions between 8-12, a prism strength of 11,4°, a number of pointing movements of 60, and a total rehabilitation time of 14 days. Patients showed improvement in FIM (mainly global and cognitive) and CBS scores, indicating a beneficial effect on activities of daily living. Furthermore, it was found that the use of prismatic adaptation can be beneficial for both left-sided and right-sided neglect syndrome.
