dc.description.abstractHlavním cílem této bakalářské práce bylo získat přehled o výskytu donor specifických protilátek (DSA) u pacientů po transplantaci ledviny. DSA hrají významnou roli v přežívání štěpu. Dalším cílem bylo zjistit zastoupení HLA alel v populaci a porovnat ho s výskytem DSA. Stanovení donor specifických protilátek probíhalo na analyzátoru Luminex s využitím screeningových imunotestů LIFECODES LSA I. a II. třídy ve Fakultní nemocnici Plzeň. Vyšetřovaný soubor tvořilo 315 vyšetřovaných pacientů z nichž mělo 91 pozitivní nález donor specifických protilátek. Z výsledků bylo zjištěno, že nejčastějšími protilátkami vyskytujícími se u pacientů po transplantaci ledviny jsou: anti-A*24:02, anti-A*23:01, anti-B*15:12, anti-B*82:02, anti-B*44:02, anti-C*17:01, anti-C*04:01, anti-DQB1*06:02, anti-DQB1*03:03, anti-DRB1*04:01 a anti-DRB1*09:01. Nejvíce zastoupenými alelami v populaci byly: A*02:01, A*01:01, B*07:02, B*08:01, C*07:01, C*07:02, DQB1*03:01, DQB1*06:02, DRB1*07:01 a DRB1*15:01. Dále bylo zjištěno, že zastoupení HLA alel v populaci nijak nekoreluje s výskytem DSA u pacientů. Pro praxi lze doporučit vyšetření epitopů antigenů proti nimž je DSA namířena.cs
dc.description.abstract-translatedThe main goal of this bachelor thesis was to obtain an overview of the prevalence of donor specific antibodies (DSA) in patients after kidney transplantation. DSA has an important role in the survival of the graft. Another objective was to determine the representation of HLA alleles in the population and compare it with the incidence of DSA. Donor-specific antibodies were determined on a Luminex analyzer using LIFECODES LSA I. and II. class screening immunoassays at the University Hospital Pilsen. The examined group consisted of 315 examined patients, which 91 of them had a positive finding of donor specific antibodies. The results showed that the most common antibodies occurring in kidney transplant patients are: anti-A*24:02, anti-A*23:01, anti-B*15:12, anti-B*82:02, anti-B*44:02, anti-C*17:01, anti-C*04:01, anti-DQB1*06:02, anti-DQB1*03:03, anti-DRB1*04:01 and anti-DRB1*09:01. The most common alleles in the population were: A*02:01, A*01:01, B*07:02, B*08:01, C*07:01, C*07:02, DQB1*03:01, DQB1*06:02, DRB1*07:01 and DRB1*15:01. The results show that there is no correlation between donor specific antibodies frequencies and HLA alleles frequencies. In practice, examination of epitopes of antigens against which DSA is directed may be recommended.en
