Etická dilemata v práci zdravotnického záchranáře

Abstract

Tento příspěvek se zaměřuje na problematiku etických dilemat v práci zdravotnického záchranáře. Věnuje se etickým dilematům v intenzivní péči, a to především těm, která souvisejí s šířením onemocnění COVID-19. Teoretická část se věnuje etice jako filozofické disciplíně, popisuje základní etické teorie a zasazuje je do medicínské praxe. V teoretické části se dále nachází deskripce základních etických dilemat intenzivní péče, a to jak pandemických etických otázek, tak etických dilemat běžného provozu oddělení akutní a dlouhodobé neodkladné péče. Praktická část obsahuje analýzu vlastního výzkumného šetření, jež jsme realizovali formou kvantitativního průzkumu metodou dotazování. Respondenty pro výzkum představovali zdravotničtí záchranáři zaměstnaní na odděleních intenzivní péče. Všichni respondenti měli zkušenost s ošetřovatelskou péčí u COVID-19 pozitivních pacientů. Zajímalo nás, jaká etická dilemata vnesla do intenzivní péče pandemie COVID-19, jaká byla četnost etických dilemat v intenzivní péči v období před pandemií a během ní. Blíže jsme se zabývali etickými otázkami alokace vzácných zdrojů. Zjišťovali jsme také postoje záchranářů k dalším etickým otázkám pandemie.This paper thesis focuses on the issues of ethical dilemmas connected with the work of paramedics. It is dedicated to the dilemmas in intense care and mainly those which are connected to the spreading of COVID-19. The theoretical part is devoted to ethics as a phylosophical discipline. It describes main ethical theories and puts them into medical practices. Another part of theoretical section is the description of fundamental ethical dilemmas of intense care, both pandemic but also ethical questions of the operation of intensive care ward. The practical part consists of the analysis of research that was made in the form of the quantitative survey done by the method of questioning. Survey respondents were medical staff employed in intense care. All respondents had experience with taking care of COVID-19 positive patients. Four health-care institutions were part of the research. The interest of the research was to discover which ethical dilemmas were brought into the intense care by the COVID-19 pandemic, how common were ethical dilemmas in intensive care before the pandemic and during the pandemic. The closer focus was on ethical questions about the allocation of rare resources. We were also finding out the stand of paramedics on other ethical dilemmas.
