Screening polohových deformit hlavy u kojenců do jednoho roku

Abstract

Cíl studie: Hlavním cílem práce bylo vyhodnotit výskyt polohových deformit hlavy u kojenců do 1 roku.Metody: Do studie bylo zahrnuto celkem 112 kojenců do 1 roku věku, kteří se narodili od 36+0/7 gestačního týdne do 42+0/7 g.t. a kteří se narodili záhlavím vaginálně nebo císařským řezem, bez použití vakuumextraktoru či kleští. V době zařazení do studie nebyli starší 1 roku věku.Všem kojencům ve sledovaném souboru byla vrámci vstupního vyšetření pořízena fotografie hlavy, ze které byly následně vyhodnoceny cefalické indexy programem Scodiac pro určení typu a závažnosti deformity hlavy. Sběr dat probíhal multicentricky ve 4 ambulantních pracovištích fyzioterapie v nestátních zdravotnických zařízeních vČeské republice vobdobí od 1.12.2020 do 30.11.2021.Výsledky: Zkojenců odeslaných kvyšetření k fyzoterapeutům bylo 63 % chlapců -tento rozdíl byl statisticky významný (P-hodnota je 0,01; <0,05). Ze získanýchdat byla prokázánastatisticky významná závislost závažnosti polohové deformity hlavy na věku kojence v době prvního vyšetření, přičemž při záchytu vpozdějších trimenonech jsou závažnější deformity než vprvním trimenonu (P-hodnota je 0,002; <0,05).Závěr: Na základě výsledků této studie můžeme konstatovat, že rizikovou skupinou pro vznik polohové deformity hlavy u kojenců je mužské pohlaví a že včasná diagnostika polohové deformity hlavy kojence předchází progresi deformace lebky kojence.

Aim of the study wad to assess the incidence of positional deformities of head in infants up to one year old. Methods: Total number of infants included in the study, aged up to one year, who were born from 36+0/7 to 42+0/7 gestational week and who were born with cephalic presentation vaginally, by Ca-esar section, without the use of vacuum-extractor or forceps, was 112. Infants were younger than one year old in the time of study entrance. Photographic pictures of the heads of infants, that were assessed by Scodiac programme for cephalic indexes to determine the type and grade of head deformity, were taken in the monitored group as a part of entrance examination. Data collection was multicentric in 4 outpatients’ workplaces of physiotherapy in private healthcare facilities in Czech Republic from 1. 12. 2020 to 30. 11. 2021. Results: From the infants sent to examination to the physiotherapists were 63 % of boys – this dif-ference was statistically significant (P-value 0,01; <0,05). Statistically significant relation of the grade of positional deformity of head to the age of infants at the time of first examination was proved from the obtained data, while the detection in the time of later trimenons showed more serious deformities than in the first trimenon (P-value 0,002; <0,05). Conclusion: Based on the obtained results we can state that the male sex is a risk group for genesis of positional deformity of head in infants and that early diagnostics of positional deformity of head in infants precedes the progression od scull deformity of the infants.
