dc.description.abstractCílem mé práce je zhodnocení výsledků intervenčního výkonu transarteriální radioembolizace (TARE) prováděného ve FN Plzeň Lochotín na Klinice zobrazovacích metod. Práce je teoretickým průvodcem přípravné a aplikační fáze samotné intervence se zaměřením na složitou logistiku, radiologickou ochranu, přípravu pacienta, indikace a kontraindikace. V praktické části hodnotím výsledky metodou RECIST jakožto odezvu nádoru na cílenou léčbu pomocí obrazové dokumentace a posuzuji dobu do progrese (progression free survival). Léčba spočívá v aplikaci radioaktivních mikročástic yttrium-90 mikrokatétrem transarteriálně přímo do nádorové tkáně jater, kde vlivem beta minus záření dochází k destrukci nádoru. Díky brachyterapeutickému účinku není ozářena okolní tkáň jater. Přínosem interpretovaných údajů bude ucelený pohled na problematiku specializované léčby onkologických pacientů s hepatocelulárním karcinomem jater či metastázami kolorektálního karcinomu.cs
dc.description.abstract-translatedThe aim of my work is to evaluate the results of transarterial radioembolization (TARE) performed at the Department of radiology and imaging sciences at the University Hospital in Plzen. The work is a theoretical introduction to the preparative and applicative part of the intervention, it is focused on complex logistics, radiological protection,preparation of the patient, indications and contraindications. In the practical part of the work I evaluate the results of the method by means of RECIST evaluation scheme as well as the tumor response to the targeted therapy using imaging documentation and I evaluate the progression free survival. The therapy is based on the aplikacation of radioactive microparticles of yttrium-90 directly to the liver tumor via percutaneous transarterial approach, the tumor mass is destructed as a result of the effect of beta minus radiation. The urrounding liver tissue is not radiated because of short range of beta radiation. The epartment of radiology and imaging sciences at the University Hospital in Plzen was the first one in central Europe performing this method in 2013 and from that time it has been carrying out this method due to close cooperation of interventional radiology and nuclear medicine and also thaks to excellent callaboration with liver surgery and oncology departaments in FN Plzeň. The benefit of my work is the complex view on the problematics of the specialized therapy of the oncologic patients with the hepatocellular carcinoma or metastases of colorectal carcinoma.en
